RSPH Level 2 Certificate Mortuary Support

Application Form

Please complete all sections 1 - 3 below on order - it is important that your take your time with this application.  It is recommended that you read the entire application in the first instance and then collect the necessary information required to complete it at a later stage. 

 

Section 1: Proceed to payments page (You will be taken to the North Tees Hospital payments web site - please keep a copy / screenshot of your payment receipt).

Section 2: Complete your personal details (remember you will need a named mentor from your organisation).

Section 3: Read the declarations​ (click the box to indicate you have read this) and then click the submit button.

Section 1: Payment Options:

 

The course fee consists are  £1800.00 total per candidate - payment options available are:

 

  • Pay by credit or debit card (please upload screen shot or receipt under 'proof of payment' when applying).  

  • Pay via purchase order (please upload copy of PO under 'proof of payment' when applying).

  • Payment over the phone.

  1. Go to the 'Make an online payment' section using the link above

  2. Select 'Other payments' link.

  3. Select the 'course fees' link form the list provided.

  4. The course name you are paying for is called ‘FD Mortuary level 2 certificate’ - enter this in the course name box.

  5. Proceed to enter your card payment details.

 

Please take a screen shot / receipt as proof of payment when using card payment 

Instructions for paying by phone:

 

You can also make a payment by calling North Tees Hospital on  01642 617617 and asking switchboard for the finance department. They can take payment over the phone.

I have read and fully understand the course requirements and eligibility criteria and commit to:

  • Completing the full course which will involve a time commitment of x3 full days teaching and x3 half days revision (online) to be spread via modular learning as outlined in the timetable supplied.

  • Completing all distance learning and assessment work (exams and assignments), set within the timeframes as required.

  • You may contact the North Tees Centre prior to your attendance to discuss any additional learning support requirements - documentary evidence may also be required for submission when you attend the North Tees Centre. The Centre is not responsible for arranging / funding any further tests which may be required – please speak to your mentor / line manager if you require additional support with testing (dyslexia for example).

The mentor:

The organisational mentor (at the organisation at which the applicant is currently employed / completing work experience) must read the following declaration.

I have read and fully understand the course requirements and eligibility criteria and commit to:

  • Act as a work based mentor to support the candidate, allow for the collection of work based evidence and to provide competency training to enable to candidate to meet the work based objectives.

  • Sharing my knowledge, skills and experience of all aspects of mortuary practice.

  • Countersigning any assignment evidence work submitted by the candidate verifying that work is true and accurate of any local processes and procedures. 

  • Candidate release for the time required to participate fully in the online training and attend local examinations.

The mentor is acting on behalf of your organisation to ensure full attendance and completion of this programme.
