Application Form

Please complete all sections 1 - 3 below - it is important that your take your time with this application.  It is recommended that you read the entire application in the first instance and then collect the necessary information required to complete it at a later stage. 

 

Section 1: Proceed to payments page (You will be taken to the North Tees Hospital payments web site).

Section 2: Complete your details (NAFD number obtained during registration on the NAFD website).

Section 3: Read the declarations​ (click the box to indicate you have read this) and then click the submit button.

Section 1: Payment Options:

 

The course fee consists of 2 PAYMENTS – these must be paid to both NAFD and North Tees – these payments are £750.00 total:

 

  • £150: NAFD fee

  • £600: North Tees fee

 

Please go to the NAFD website to make their payment of £150.

 

£600 payments to North Tees can be made online using the link below:

Just go to the 'Make an online payment' link and then select the 'course fees' link form the list. The course you are paying for is called ‘NAFD FFSO COURSE’ and then enter your payment details.

 

You can also make a payment by calling 01642 617617 and asking switchboard for the finance department. They can take payment over the phone. 

 

Please take a screen shot / receipt as proof or payment.

I have read and fully understand the course requirements and eligibility criteria and commit to:

  • Completing the full course which will involve a time commitment of 2 days (online) to be spread via modular learning as outlined in the timetable supplied.

  • Completing all distance learning and assessment work, set within the timeframes as required.

  • Completing a portfolio of evidence for final assessment by the North Tees Centre.

  • You may contact the North Tees Centre prior to your attendance to discuss any additional learning support requirements - documentary evidence may also be required for submission when you attend the North Tees Centre.

  • The Centre is not responsible for arranging / funding any further tests which may be required – please speak to your mentor / line manager if you require additional support with testing (dyslexia for example).

  • All assessed work must be completed within 6 months of completion of theory taught lessons.
