Application Form

Please complete all sections 1 - 3 below - it is important that your take your time with this application.  It is recommended that you read the entire application in the first instance and then collect the necessary information required to complete it at a later stage. 

 

Section 1: Complete your details (NAFD number obtained during registration on the NAFD website).

Section 2: Read the declarations​ (click the box to indicate you have read this) and then click the submit button.

Section 3: Proceed to payments page (You will be taken to the North Tees Hospital payments web site).

I have read and fully understand the course requirements and eligibility criteria and commit to:

  • Completing the full course which will involve a time commitment of 2 days (online) to be spread via modular learning as outlined in the timetable supplied.

  • Completing all distance learning and assessment work, set within the timeframes as required.

  • Completing a portfolio of evidence for final assessment by the North Tees Centre.

  • You may contact the North Tees Centre prior to your attendance to discuss any additional learning support requirements - documentary evidence may also be required for submission when you attend the North Tees Centre.

  • The Centre is not responsible for arranging / funding any further tests which may be required – please speak to your mentor / line manager if you require additional support with testing (dyslexia for example).

  • All assessed work must be completed within 6 months of completion of theory taught lessons.

Section 3: Payment Options:

 

Please ensure you have completed your personal details above and submitted your application to us first before proceeding to payment. We will cross-reference all course applications received against any payments received.

Cost for each candidate is £600 - Click here to make payment (s) to North Tees and Hartlepool NHS Trust - this will open to an external web site.

Once on the NHS site navigate to: 'Make an online payment' > 'Other payments' > 'Course fees' (remember the course title name is: Frontline Funeral Service Operations Training).

 

Candidates may make two payments of £300 (final payment must be made before completion of training – certificate will not be issue until final payment received). Please contact darren.makin@nhs.net for further information.

Once we have received your online application, we will send your course joining instructions via the email supplied above within 5 working days.
